Family Tree Assisted Living

Welcome to Family Tree Assisted Living of West Point

We have been open since May 2006. In the short time we have been open; we have had an overwhelming response from the community and health care providers.

Our Administrator as well as our Wellness Director each has many years of experience in elderly care. They have been through two state surveys with Family Tree and have had a perfect score with each survey.

We are a Type I and a Type II Assisted Living, as well as Independent living. We offer apartment style living for those who wish to enjoy our first class amenities and socialization of our community, but do not yet require assistance in daily living.

Please explore this website for more information about our facility.

Or call 801- 775-TREE with any questions

We are proud to serve the North Western corridor of Davis County for needs in Type I and Type II Assisted Living.

Independent Living is also available in our Patio Style Apartments for those who wish to enjoy the first class amenities and socialization of our community, but do not yet require assistance in daily living.

Many thanks to our Administrator Sheila and our LPN Julia for hand picking and training an incredible staff. Our standard for care has been set very high by our energetic and well trained CNA's and we promise to continue this high quality of service.  We also promise to be among the most affordable care option in Northern Utah.

Please explore our website to learn more about this brand new top-of-the-line facility in beautiful West Point City or Contact Us with any questions.

About the Owners and Creators of Family Tree
Family Tree was envisioned by Shane Ericksen.  Shane is the son of Dr. Corey Ericksen who practices at Westside Medical in Clinton, Utah.  Dr. Ericksen and family built Country Pines Assisted Living in Clinton in 1994.  Since then, it has been a pillar in the community.  Country Pines has an outstanding reputation with caregivers, health care professionals, and of course, our residents and families.

We are excited to bring this continuum of care full circle by now offering Western Davis County Type II Care. We will ensure that the same philosophies, commitment to the highest quality of service, and fantastic staff will be duplicated at Family Tree at West Point.
